New and established patients:

- Please arrive at the office 10 minutes early to allow for filling out forms, collecting copays, collecting payment of services not covered by your insurance, presenting cards and other needed patient information. A patient information sheet may be downloaded here and filled out prior to your office visit for your convenience.

- Before coming to our office contact your insurance company or health plan to verify our physicians are in your insurance network to prevent you from getting a bill for out of network services.

- Should your insurance or health plan require a referral for a specialist one will be mailed to you after several days. Only in emergencies will a referral be issued on the same day as your office visit.

- Insurance cards must be presented at every office visit along with applicable copays. Patients will be rescheduled if these requirements can not be fulfilled at the time of the service.

- Patient statements are billed monthly. Payment is expected in our office within 30 days of receiving the statement. If you have a health plan, which is in our network we will bill your insurance and you will not get a bill until a response has been received by your insurance carrier. Once we receive a response, you will be billed for any unpaid expenses.

- There will be a fee for copying medical records, writing letters, or filling out forms since these services are a non-covered benefit of your insurance plan.

- Visa and MasterCard are welcome.

- We are handicapped accessible.
